Please fill out a Dog Tag Application Form online. Please note that your dog's rabies certificate must be valid at the time of application. If your dog has been spayed or neutered since the rabies vaccination, please include a copy of that certificate too.
Renewal period is November 1 to January 31 each year. You must send a valid rabies certificate for each dog. The tag will expire same year as the certificate. Optional multi-year tags available if eligible. Certificates and tags will be returned by mail. The tag must be on your dog by January 31 of every year. County and state code require all dogs to be registered within 90 days of vaccination.
